Medical Emergency Notice: Telehealth is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use this platform for emergency situations.

This Telehealth Informed Consent explains the nature, benefits, risks, and limitations of receiving healthcare services through a telehealth platform. By proceeding with your intake and consultation, you acknowledge that you have read, understood, and voluntarily consent to the terms described below.

Note: Healthcare services provided through Vita Stack Health are delivered by independent licensed medical providers through our clinical partner network. Your treating provider may present you with an additional informed consent specific to their practice during your consultation. Both consents apply to your care.

What Is Telehealth?

Telehealth refers to the delivery of healthcare services using electronic communications, information technology, and other digital means to connect patients and licensed healthcare professionals who are not in the same physical location.

Through our platform, telehealth services may include:

  • Asynchronous consultations (review of your health questionnaire by a provider)
  • Video consultations with a licensed provider
  • Secure messaging between you and the care team
  • Electronic prescribing and prescription management
  • Ongoing remote monitoring and follow-up

Benefits of Telehealth

Telehealth offers a number of advantages, including:

  • Access to licensed medical care from the comfort of your home
  • Elimination of travel time and waiting room delays
  • Faster access to consultations and prescription management
  • Continuity of care through digital follow-ups and check-ins
  • Access to providers across your state regardless of your location

Risks & Limitations of Telehealth

Like any form of medical care, telehealth has potential risks and limitations you should be aware of:

  • Technical limitations: Poor internet connections, software errors, or equipment issues may interrupt or affect the quality of your consultation.
  • Limited physical examination: Your provider cannot physically examine you. Certain conditions may require an in-person evaluation that telehealth cannot replace.
  • Information security: While we use encryption and HIPAA-compliant technology, no digital transmission is completely risk-free. There is a small risk that information could be intercepted.
  • Provider judgment: Your provider may determine that your condition is not appropriate for telehealth and may refer you to in-person care.
  • Delays: Technical issues may occasionally delay your consultation or care.
  • Medication limitations: Not all medications can be prescribed via telehealth in all states.

Privacy & Security

We take the privacy and security of your health information seriously. All telehealth consultations and communications through our platform are conducted using HIPAA-compliant, encrypted technology.

Your health information will only be shared with:

  • Your assigned licensed provider and their affiliated medical group
  • Our partner compounding pharmacy for prescription fulfillment
  • Other parties as described in our Privacy Policy

You should conduct your telehealth consultation in a private setting to protect your own confidentiality. We are not responsible for breaches of privacy that occur on your end (e.g., others overhearing your consultation).

Your Rights as a Patient

By using our telehealth services, you retain the following rights:

  • Right to withdraw consent: You may withdraw your consent to telehealth services at any time without penalty. Withdrawing consent will not affect care you have already received.
  • Right to request in-person care: You may request a referral to an in-person provider at any time if you prefer or if your provider recommends it.
  • Right to ask questions: You have the right to ask your provider any questions about your diagnosis, treatment plan, medications, or alternatives before proceeding.
  • Right to refuse treatment: You are never obligated to accept a recommended treatment or prescription.
  • Right to access your records: You may request a copy of your medical records at any time by contacting support@vitastackhealth.com.

Independent Provider Relationship

Healthcare services provided through this platform are delivered by independent licensed medical providers who are not employees or agents of VitaStack LLC. The prescribing provider operates independently and is solely responsible for all clinical decisions, including whether to prescribe medication.

VitaStack LLC does not practice medicine and does not interfere with or direct the clinical judgment of any provider.

A prescription is never guaranteed. Your provider will make an independent determination about whether treatment is appropriate based on your individual health information.

Compounded Medications

Medications prescribed through this platform may include compounded GLP-1 medications such as semaglutide or tirzepatide. You acknowledge and understand that:

  • Compounded medications are not FDA-approved
  • Compounded medications are not the same as brand-name drugs such as Ozempic®, Wegovy®, or Mounjaro®
  • Compounded medications are prepared for you individually by a state-licensed, PCAB-accredited compounding pharmacy
  • As with all medications, there are potential side effects and risks which your provider will discuss with you
  • You should report any adverse reactions to your provider or, in an emergency, call 911

Electronic Consent & Communications

By proceeding with your intake form and consultation, you:

  • Consent to receive healthcare services via telehealth as described above
  • Consent to electronic communications including email, secure messaging, and SMS for care-related updates
  • Acknowledge that your electronic submission carries the same legal weight as a written signature
  • Confirm that you are at least 18 years of age
  • Confirm that you are located in a state where our services are available
  • Confirm that the health information you provide is accurate and complete to the best of your knowledge

This consent remains in effect for all future telehealth services you receive through Vita Stack Health unless you withdraw it in writing. You may withdraw consent at any time by contacting support@vitastackhealth.com.
